Terms and Conditions: -                                              

1.    Members of ZMS card holder can avail discount up to 30% on services provided by our empaneled service providers.

2.    Discount on pharmacies shall be subjected to only on medicines and surgical items, not on cosmetics or other products.

3.    ZMS card will be valid for one year starting from the date of registration of card. Please check the dates on ZMS card for validation of your membership.

4.    Same ZMS health card cannot be used for another year after expiry of the card in any case.

5.    Card member must present ZMS card physically every time while availing the facilities.

6.    ZMS card is valid only when used by the listed members on ZMS health card themselves.

7.    This card cannot be used by third party, cannot be copied, altered or modified to avail services if found so then membership will be terminated with immediate effect and legal action will be initiated against the defaulter.

8.    Empaneled service provider may ask for Aadhaar card of members in verification process of authentic and anti-fraudulent.

9.   ZMS card shall provide discounts on only those services, medicines and medical equipment which are strictly subjected to the availability at their respective service providers.

10. ZMS card cannot be used in any means to settle bills.

11. Zegham Medical Services Pvt. Ltd. will not be liable in case of ZMS card being not been honored by any empaneled service provider, it shall remain the sole responsibility of that particular empaneled service provider. But if card member lodge a complaint in written about such incidence then we may cancel the empanelment of the institution, only after raising an adequate enquiry.

12. Similarly, in the event of any complaint of fraud, misuse or abuse in concern with ZMS card then membership can be terminated after raising an adequate enquiry.

13. In the event if ZMS card is lost then member must immediately inform us in writing and shall bear Rs. 350/- fees for a new card to be issued.

14. Empaneled service providers list may change from time to time, as addition and removal of service providers will be a routine work of company. Additional and removal of health institutions can be uploaded from our website.

15.  Every ZMS card holder will be bound and agree on all mentioned terms and conditions and terms & conditions added from time to time by the company.

16. If any person or agency or Common service center or employee found copying, duplicating, misusing in any sense of ZMS health card, a strict and legal action will be taken against them.

17.  All disputes regarding Zegham Medical Services Private Limited are subjected to the jurisdiction of district court Kullu, H.P.
